MSHSAA Bylaw 3.8.1 and 4.5.4 now allows a physical exam certified by a medical professional (MD/DO/ARNP/PA/Chiropractor) to be valid for a duration of two years (730 days) from date of issue (unless a physician specifically notes on the PPE that the physical is only valid for one year).

MSHSAA PRE-PARTICIPATION DOCUMENTATION (Annual Requirements):  While the MSHSAA PPE Form is now valid for a two year period from the date of the physical examination, all other Pre-participation Documents must still be completed/updated annually for every student athlete.  MSHSAA PRE-PARTICIPATION DOCUMENTATION THAT MUST BE COMPLETED ANNUALLY:  Medical History (pg 1-2), Parent Permission-Insurance Verification (pg 3), Student Agreement (pg 4), Concussion Education Materials-Parent/Student Signature (pg 4), Emergency Contact Information (pg 4).

If you are a student that turned in a completed physical last school year (2018-19), you only have to fill out and return the following form:

MSHSAA Pre-Participation Physical Annual Requirement Form

If you did not have a physical on file with the Athletic Office, then you need to fill out the form and bring it with you when you are signed up for on June 3 or June 5 at the Elementary.

All students entering grades 7-12 that are in marching band, cheer, football, softball, cross country, golf, basketball, track, and baseball must have a completed physical on file before participating.
