The Salisbury School District in conjunction with the Family Health Center and the Chariton County Health Department will be offering free physicals for students entering grades 7-12 for the 2018-19 school year. The dates for the physicals are Tuesday, May 29, and Wednesday, May 30. These physicals will meet the MSHSAA requirement for participation in sports, band, and cheerleading.
Steps to Getting your Physical
1) Sign up for a time by clicking the following link:

2) Download and completely fill out (page 1, 3, 4) the following physical form:

3) Bring your completed form on the day and time of your physical to Salisbury High School Library. Please enter on the West side of the school by the library.
4) After your physical is complete you will be given a copy to keep for your records.
